Rating scales, completed and scored before the visit.
Readily Health sends ADHD questionnaires to patients, parents and teachers, collects the fee before anyone starts, and scores each form with the instrument's published rules. You open a result, not a stack of paper.
Now onboarding a small number of practices.

Ready for review
4 of 6 answers in the shaded range. Screening result: further clinical evaluation indicated.
This is a screening result, not a diagnosis.
One order. Every form back, paid and scored.
Monday
You order
Pick the patient and the questionnaires. Readily Health suggests forms by age and only lists those cleared for use. The payer gets a secure link by email.
Tuesday
The payer pays
The adult patient, or a parent for a child, pays the assessment fee by card before the first question appears. Teacher links go out only after that.
Wednesday
Respondents complete
Each person answers on their own phone or computer, after confirming the patient's date of birth. Answers save as they go.
Thursday, appointment
You review
Every form is scored the moment it is submitted. Parent and teacher forms sit side by side. Mark it reviewed, add a private note, export a PDF for the chart.
What changes for your practice
- Nothing to chase.
- One order sends every link. Reminders, resends and expiry are handled from your dashboard, and you can see at a glance who has paid, started or finished.
- Paid before it starts.
- No question is shown until the assessment fee is paid. No unpaid forms, no invoices, no awkward call at checkout. Refunds stay in your hands.
- Scored the way the form says.
- Hand tallies slip, and tools that pool symptom counts across items can flag more patients than the instrument would. Readily Health applies each instrument's published thresholds, and is tested at every one of them.

Saved
Easy on the people filling it in.
Parents, teachers and adult patients get a short, phone-first form with large answer targets and nothing extra to sign up for.
- Works in any phone browser. No app to install, no account to create.
- A date-of-birth check comes before anything about the patient is shown. Five wrong tries lock the link.
- Answers save as they go, so a parent can stop at school pickup and finish that evening.
- Teachers get their own link and access code, and never see a payment screen.
Only forms you're allowed to use.
Every questionnaire carries its licence. If the licence does not allow commercial use, the form stays off until written permission is on file. Item wording is copied exactly from the official form.
| Questionnaire | For | Status |
|---|---|---|
| ASRS v1.1 Screener | Adults 18 and over, self-report, 6 questions | Available |
| Supplemental history form | Written by your practice: onset, settings, history, medications | Available |
| NICHQ Vanderbilt, parent and teacher | Children 6 to 12 | Once licensing is in place |
| ASRS-5 and ASRS v1.1 Symptom Checklist | Adults 18 and over | Once licensing is in place |
Patient details stay where they belong.
Readily Health is built for HIPAA workflows, and makes no exceptions for convenience. No analytics, no tracking pixels, on this site or in the app.
-
Emails say a form is waiting. They never name the patient, the questionnaire or a condition.
-
The payment processor sees an amount and an order number. Nothing about the patient.
-
Invite links carry no patient information, and their secret part never reaches a server log.
-
Every staff account uses two-step sign-in. Sessions end after 15 minutes without activity.
-
An append-only audit log records sign-ins, result views, PDF exports and payments.
-
Each practice's records are isolated at the database level, with tests that prove it on every change.
Readily Health screens. Your clinicians decide.
Every result screen and PDF says it plainly: this is a screening result, not a diagnosis. A diagnosis requires a clinical evaluation.
Questions practices ask.
Who pays, and when?
The adult patient, or for a child the parent or guardian, pays by card before starting. Teachers never pay. The fee goes to your practice, and refunds are issued by your practice.
What does the patient see after submitting?
A thank-you screen. Results go to your practice only, so your clinicians can discuss them in context.
Which ages does it cover today?
Adults, with the ASRS v1.1 Screener and your own supplemental history form. Child and teacher rating scales are switched on as licensing is completed. Until then, child orders can include the supplemental form.
How much does it cost?
Pricing is set with each practice during onboarding. Email us and we'll walk you through it.
Does it connect to our EHR?
Not yet. Each result exports as a PDF for the chart, with the instrument's attribution and the screening statement on every page.
Bring your next assessment in ready.
We're onboarding a small number of practices. Tell us about yours and we'll set up a walkthrough with your clinicians and front desk.
